QMHP / Intensive In-Home Worker

The Intensive In-Home Worker will plan and implement interventions for children and their families, specifically focusing on children with DSM 5 Diagnosis. The IIH Worker will facilitate assessments and produce individualized service plans.  Some additional duties will be to provide children and family support through individualized interventions to include; anger management, reduce impassivity and aggression, grief issues, parenting education, and increasing positive coping skills.  Additionally, will provide case management, service coordination, and emergency response and crisis intervention as needed.

Supervision for Residents in Counselor and Supervisee’s in Social Work offered free of charge.

Qualifications are:
• Bachelor’s degree in human services field (social work, psychology, sociology, education, and counseling)
• Preferred: one year of clinical experience working directly with clients, children and adults who have mental health issues.
• QMHP or QMHP-T and must be eligible for QMHP status with Virginia Board of Counseling.

Behavior Technician/Registered Behavior Technician (RBT)

Community Registered Nurse Coordinator

 Experience:

 Registered Nurse to support adults with Intellectual Disabilities in a community residential setting. The position is part of a collaborative team focused on promoting health, independence and meaningful daily experiences for individuals living in the community.   A successful candidate will have at least 5 years experience with adults. Prior experience with adults who are diagnosed with intellectual disabilities is preferred.  The successful candidate will demonstrate strong clinical skills, effective collaboration, and a commitment to person-centered care.

Education requirements:

 Qualified candidate will be possess a Bachelor of Science in Nursing and hold an active license with the Virginia State Board of Nursing.

  • Will complete comprehensive health assessment fulfilling requirements as outline in Medicaid manual according to Services and maintain assessments and updates to assessments in agency EMR
  • Create Individualized care plans that comply with Developmental Disabilities waiver criteria and maintain records in WAMS (Waiver Management System) as well as agency Electronic Medical Record
  • Monitor medical status changes, educate and train Direct Service Providers (DSP)when changes occur and maintain documentation of changes in WAMS as well as agency EMR
  • Attend Individualized Service Plan Meeting for assigned individuals who have been identified as having hronic medical needs
  • Collaboration with hospitals for admission and discharge
  • Review lab tests and other diagnostic reports and recommend follow-up action if needed
  • support instructions identified in the individual support plan address identified health risk and medical diagnosis
  • Coordinate services with in home skilled nursing and hospice nursing agencies to ensure care duties are defined and understood
  • Review Health visit and ER discharge summaries ensure DSP understanding and implementation of medical professionals’ recommendations
  • Delegate nursing activities to DSP as needed
  • Travel to individuals residences to complete home visits for purpose of DSP training and on-going needs assessments of supported individual
  • Complete DSP training and ensure competency as related to medical protocol including but not limited to g-tube care, seizure protocol, colostomy care, adaptive equipment, wound care, pressure sores, and uti prevention and care
  • Provide education regarding Dietary needs and plans as they arise
  • Ensure compliance with regulatory entities to include Department of Behavior Health and Developmental Services, Virginia Administrative Code, Department of Medical Assistance and Virginia Board of Nursing in relation to care plan development, implementation, and documentation
  • Support supervisors who oversee DSP’s who work with medically complicated individuals
  • Coordinate with community medical professionals to develop protocols and ensure that protocols are documented in a manner that supports DSP understanding and implementation of ordered interventions
  • Monitor and review Critical Incident reports (CHRIS) and other quality assurance tools as decided upon
  • Collaborate with treatment team to complete authorization requests for skilled nursing as well as ongoing reauthorization when needed
  • Complete documentation for nursing support provided as identified in the nursing care plan and maintain documentation in the EMR
  • Review , monitor and oversee medication administration (MAR) in accordance with DBHDS regulations, agency policies and applicable licensing requirements
  • Review and update policies and procedures as needed
  • Remain on call for DSP’s for the purpose of supporting the individual medical needs
  • Other related duties as defined by the agency President in response to licensing requirements and organizations needs or operations
